Licensing Music Recordings For Film, Video, and Television (Master and/or Publishing)

Thank you for your interest in licensing recordings from the Kosmic Music catalogue. Attached you will find detailed instructions about how to complete the applicable forms (Instructions for completing forms). The Master Use Request Form and The Publishing Use Request Form contains all of the information needed to process your request. PLEASE FILL OUT THE FORMS COMPLETELY, INCLUDING ALL CONTACT INFORMATION AS WELL AS A FAX NUMBER AND E-MAIL ADDRESS. Failure to do so, will prevent us from processing your request in a prompt manner. Once you have completed this form, please send it via fax to the attention of Ivette M. Mares (see below for contact info).

Upon Kosmic's receipt of your completed forms, we will commence the approval process. We will contact you once we have secured an approval or denial on your request. Please allow ample time for the clearance process.
